About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our Early Intervention in Psychosis Team at Coventry and Warwickshire Partnership NHS Trust.

Job Summary

The successful candidate will be responsible for providing high-quality care coordination services to individuals experiencing first episode or early psychosis, as well as those at risk of mental health issues. This will involve conducting detailed assessments, developing and implementing care plans, and working collaboratively with other healthcare professionals to ensure the best possible outcomes for our service users.

Main Responsibilities
  • Conduct initial assessments and develop care plans in collaboration with service users, their families, and other healthcare professionals.
  • Provide ongoing support and coordination of care to ensure service users receive the necessary interventions and services.
  • Work closely with other team members to ensure a comprehensive and coordinated approach to care.
  • Identify and manage risk, utilizing the Steve Morgan tools and positive risk-taking strategies.
  • Maintain accurate records and reports, including the use of My Care Record.
Requirements
  • Relevant professional qualification (e.g. RMN, DipCOT).
  • Current unrestricted registration with HCPC/NMC.
  • Specialist knowledge across a range of procedures and theory.
  • Professional knowledge acquired through degree/diploma.
  • Relevant training and experience in specialist short courses.
  • Evidence of continuing professional development.
  • Substantial experience in a community mental health care setting/psychosis.
  • Developed specialist knowledge gained within a similar environment over a considerable period of time.
  • Significant experience of undertaking assessment, formulation, risk assessment, and positive risk management.
  • Experience of safeguarding and investigations.
  • Proven experience of people management.
  • Full driving license and access to a motor vehicle for business use.
