About the Role

This is a challenging yet rewarding position that requires a positive and enthusiastic individual who can work independently and as part of a team. The successful candidate will have significant experience in community care coordination, an interest in promoting holistic health and wellbeing, and strong IT skills.

Main Responsibilities
  • Conduct comprehensive initial assessments to identify at-risk mental states or first episode psychosis, incorporating the CAARMs assessment.
  • Develop and implement individualized care plans, working collaboratively with service-users and their families.
  • Support service-users to achieve their goals, addressing difficulties with housing, finances, vocational activities, and social inclusion.
  • Work creatively with other team members and independently to support service-users.
  • Identify and manage risk, utilizing the Steve Morgan tools, including positive risk taking.

Person Specification
  • Essential Qualifications:
    • Relevant Professional Qualification (e.g. RMN, DipCOT)
    • Current unrestricted registration with HCPC/NMC
    • Specialist knowledge across a range of procedures and theory
    • Professional knowledge acquired through degree/diploma
    • Relevant training and experience specialist short courses
    • Evidence of continuing professional development
  • Essential Experience:
    • Substantial experience in a Community Mental Health care setting/Psychosis
    • Developed specialist knowledge gained within a similar environment over a considerable period of time
    • Significant experience of undertaking assessment, formulation, risk assessment, and positive risk management
    • Experience of safeguarding and investigations
    • Proven experience of people management
  • Essential Other:
    • Must have full driving licence and access to a motor vehicle for business use
