Role Overview

This is a full-time permanent position with a salary of £34,637 - £41,564 per annum, offering a great work-life balance through flexible working arrangements.

Job Responsibilities

As a key member of the Integrated care team (LAP), you will work closely with colleagues to provide holistic support to residents, addressing their physical disabilities and cognitive decline. Your responsibilities will include:

  • Undertaking assessments and developing interventions for residents with complex needs
  • Collaborating with other professionals to achieve identified outcomes
  • Managing a caseload and prioritizing tasks effectively
Requirements

We are seeking a qualified individual with experience in community or hospital settings. The ideal candidate should have:

  • A good understanding of IT systems, including MS Office, Work, and Excel
  • A valid UK driving license
