Advanced Clinical Care Specialist

We are looking for a skilled Advanced Clinical Care Specialist to join our team at Primarycare24. As a member of our Out of Hours Services team, you will play a vital role in delivering high-quality patient care in a fast-paced environment.
About the PositionThis is a full-time opportunity for a qualified APNP who can work autonomously, supported by GP colleagues. You will be responsible for assessing, diagnosing, and managing patients presenting with undiagnosed or undifferentiated illness via telephone consultations or face-to-face assessments.
The ideal candidate will have strong clinical skills, including critical thinking and decision-making, and be able to interpret complex medical information. Key responsibilities include:
- Undertaking face-to-face and telephone triage consultations
- Interpreting pathology/radiology results/reports
- Prescribing and reviewing medication
- Planning, implementing, and evaluating effective management plans for patients
We offer a competitive salary of £60,504 to £72,293 per annum, plus unsociable enhancements, as well as a range of benefits including:
