**Opportunity Overview**

We are seeking an experienced Advanced Nurse Practitioner to join our team at Service Care Solutions Ltd, working within our Home Visiting Service based in the North West. This is a rewarding opportunity for a skilled healthcare professional to provide high-quality nursing services to patients in their own homes.

**Role Summary**

The successful candidate will be responsible for delivering evidence-based nursing services as part of our multi-disciplinary team, working autonomously and taking ownership of patient care. Key responsibilities include:

  • Developing and implementing individual treatment plans for acute and non-acute conditions
  • Providing routine nursing care to patients, adhering to clinical guidelines and national standards
  • Prescribing medication in accordance with PGD and own scope of practice, reviewing medication for effectiveness and need

**Required Skills and Qualifications**

To be successful in this role, you will require:

  • A current nursing registration and relevant postgraduate qualifications
  • Experience working as an Advanced Nurse Practitioner or similar role
  • Excellent communication and interpersonal skills
