At Remedy Social Work, we are seeking a qualified and experienced Social Worker to join our team in Wirral Adult Services. As a key member of our integrated team, you will play a vital role in supporting the delivery of health and social care functions for adults and carers.

About the Role

The successful candidate will be responsible for assessing, accessing, and implementing a range of social care services, providing guidance and advice to adults with complex needs. You will work closely with the Multi-Disciplinary Team (MDT) to plan, monitor, and evaluate work, ensuring the best possible outcomes for service users.

We offer a competitive salary of £35,000 - £45,000 per annum, depending on experience, plus a referral bonus of up to £250 per person placed. Our commitment to professional development means that you will have access to regular training and support to enhance your skills and career.

  • Work as part of an integrated and supportive team, planning, monitoring, and evaluating work with the MDT.
  • Provide social work input and support to adults/carers/families, promoting self-determination, choice, and autonomy.
  • Support the Multi Agency Safeguarding Hub (MASH), Multi Agency Risk Assessment Conferences (MARAC), and safeguarding processes.
  • Carry out needs-led assessments, work of a complex nature, care and support planning, and reviews.
  • Operate and practice in line with the Professional Capabilities Framework (PCF) and model these professional behaviours to others.
