Care Coordinator

About the Role:
We are seeking a skilled and compassionate Care Coordinator to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a Care Coordinator, you will play a crucial role in supporting children with severe learning disabilities and complex needs. You will work collaboratively with families, carers, and professionals to develop and implement personalized support plans.
Responsibilities:
- To undertake single assessments and provide support to children and young people with complex disabilities
- To work closely with families and carers to identify their goals and aspirations
- To develop and implement tailored support plans that promote independence, choice, and control
- To contribute to safeguarding enquiries and court proceedings as required
Requirements:
- Social Work qualification (Degree in Social Work, DipSW or equivalent)
- Registration with Social Work England
- Evidence of continuing professional development (CPD)
- Able to demonstrate practice that supports the core principles of the College of Social Work and Families and Wellbeing Directorate
Benefits:
