Job Summary

The Advanced Clinical Practitioner (ACP) will provide specialist healthcare to patients in Healthier West Wirral care homes. This role involves independent clinical decision-making, assessment, and diagnosis of patients within our care homes. The ACP is accountable for appropriately referring patients to relevant colleagues or specialties for further review and/or investigations.

Main Duties
  • Visits to care homes as part of proactive care rounds to deliver routine follow-ups and reviews of recently discharged patients.
  • Create and review personalized care plans for each resident, identifying those at risk of preventative/proactive admission avoidance.
  • Conduct proactive care planning with the multidisciplinary team, including DNACPRs, Advance Care Plans, RESPECT, and Emergency Health Care Planning to improve care and reduce avoidable admissions.
  • Manage a caseload of complex clients using advanced problem-solving skills and clinical reasoning.
  • Maintain accurate documentation and patient records, liaising with supervising GPs and families as required.

Healthier West Wirral PCN supports over 70,000 patients across six practices. We are an ambitious organization that has built a large PCN team, including Physician Associates, Pharmacists, Care Coordinators, and Paramedics.

Job Responsibilities
  • Clinical responsibilities include direct assessment, examination, investigation, and diagnosis of patients within their area of work. Appropriately treat patients, resulting in safe management and referral or discharge of patients with undifferentiated presentations.
  • Manage own caseload and clinical priorities according to agreed protocols and working practices.
  • Participate in multidisciplinary clinics, ward rounds, patient reviews, and team meetings.
  • Undertake diagnostic procedures and clinical investigations related to patient plans of care.
Requirements

The post holder must be a registered healthcare professional with an MSc in Advanced Clinical Practice or equivalent. They should be an independent Non-Medical Prescriber registered with their relevant professional body. This role requires practice at an advanced level, demonstrating in-depth knowledge and competence in all aspects of acute, community, primary, and social care medicine.

Estimated Salary

£55,000 - £65,000 per annum, depending on experience.
