Advanced Clinical Practitioner

The role of the Advanced Clinical Practitioner involves delivering high-quality patient-centered care in a variety of settings, including care homes and the community. The successful candidate will be a registered healthcare professional with an MSc in Advanced Clinical Practice or equivalent, as well as experience in Non-Medical Prescribing.

Key Responsibilities

• Conduct proactive care rounds in care homes, reviewing routine follow-ups and patients recently discharged from hospital.
• Create and review personalized care plans for residents, identifying those at risk of preventative admission.
• Collaborate with the multidisciplinary team to develop proactive care planning, including DNACPRs, Advance Care Plans, and Emergency Health Care Planning.
• Maintain accurate patient records, liaising with supervising GPs and other healthcare professionals as required.

About Us

Healthier West Wirral Primary Care Network supports a population of over 70,000 patients across six GP practices. We are an ambitious organization with a large PCN team, including Physician Associates, Pharmacists, Care Coordinators, and Paramedics.

The ideal candidate will have a strong background in healthcare, with experience in Non-Medical Prescribing and advanced clinical practice. They will also possess excellent communication and teamwork skills, with the ability to work effectively in a multidisciplinary setting.
