We are seeking a skilled and compassionate Mental Health Specialist Care Coordinator to join our team at Cheshire and Wirral Partnership NHS Foundation Trust.
About the RoleThe successful candidate will be responsible for delivering high-quality care to adults with mental health problems, working closely with multidisciplinary teams to ensure seamless service delivery.
Key Responsibilities- Carrying a clinical caseload and providing psychological interventions at a one-to-one and group level.
- Developing and implementing specialist programmes of care, care packages, and providing specialised advice in relation to care.
- Working closely with commissioners and care providers to ensure effective service delivery.
- Educated to diploma/degree level and registered with the appropriate professional body.
- A minimum of 2 years' experience working with adults with mental health problems with a complex nature.
- In-depth knowledge and experience of working within the recovery focused philosophy framework.
- An annual salary of £32,000 - £43,000 depending on experience.
- Opportunities for continued professional development and training in Structured Clinical Management and other therapeutic models.
- A supportive and friendly multi-disciplinary team environment.
