About the Role

We are seeking a skilled Community Health Specialist to join our team, providing high-quality care to patients in need of urgent attention. This is an excellent opportunity for a motivated and compassionate individual to make a real difference in the lives of others.

As a Community Health Specialist, you will work closely with our multidisciplinary team to deliver patient-centered care, focusing on prevention, early intervention, and effective management of acute illnesses. Your expertise will be essential in assessing patients' needs, developing personalized care plans, and coordinating services to ensure seamless transitions between community and hospital settings.

The ideal candidate will possess excellent communication skills, empathy, and a strong commitment to delivering exceptional patient care. A degree in nursing or a related field, along with current registration with the NMC, is essential for this role. We also welcome applicants with relevant experience in emergency nursing, primary care, or acute medicine.
