Job Description

As a Community Health Support Specialist with HHP, you will work closely with our team to provide support and resources to individuals in need.

Your responsibilities will include attending training, learning and operating the patient intake/management system, and assisting in its development and improvement.

You will also complete HHP patient advocacy system monitoring, provide exceptional customer service skills at all times, and commit to a regular schedule.

Furthermore, you will be knowledgeable of and abide by the HHP Confidentiality Policies and Procedures.

This position offers a competitive salary of $50,000 - $60,000 per year.
