Job Summary

We are seeking a highly skilled and experienced Nurse to join our team as a Clinical Nurse Specialist in Memory Assessment Services. The successful candidate will be responsible for supporting the titration and monitoring of medication, as well as monitoring physical health observations within our Diagnostic Service.

Main Responsibilities
  1. Lead a small caseload of patients as the primary professional.
  2. Oversee medication titrations and medication monitoring.
  3. Liaise with GPs regarding medication changes and ensure medications are supplied.
  4. Participate in the continuous improvement of the service and the implementation of operational and clinical policies.
  5. Promote opportunities for local service development.
  6. Adhere to professional conduct, professional standards, and national, trust, and local policies and procedures.
  7. Demonstrate the sharing of knowledge, skills, and attitudes with others.
  8. Participate in multi-disciplinary meetings to improve clinical practice.
  9. Travel across the Blackpool, Fylde, and Wyre areas to undertake home visits, requiring the use of a car, with travel expenses paid.
  10. Demonstrate an understanding of our trust values.

Person Specification

Education/Qualifications

Essential

  • Registered Mental Health Nurse; Degree Level or equivalent knowledge and experience
  • On-going registration with the NMC
  • Compliance with regulatory bodies and codes of professional conduct

Desirable

  • Mentorship qualification
  • Sign-Off Mentor qualification
  • Knowledge of current key NHS developments and policies and implications for service delivery

Knowledge

Essential

  • Experience in dementia and cognitive impairment
  • Older adults experience
  • Medications used for Dementia Cognitive problems and Dementia diagnoses
  • Knowledge of current mental health legislation/policy

Desirable

  • Experience in memory assessment service
  • Medication titration/knowledge
  • Facilitating groups

Experience

Essential

  • Managing caseload /working under pressure
  • Experience of effectively working with service users and carers
  • Experience working as an independent and autonomous practitioner
  • Experience of delivering evidence-based interventions in a time-limited framework
  • Evidence of assessment across a broad range of mental health problems
  • Evidence of continuing professional development

Desirable

  • Experience in memory assessment service
  • Community experience
