We are seeking a highly skilled and experienced Nurse to join our team as a Titration Nurse-Memory Assessment Services Specialist. This is an exciting opportunity to work with our Diagnostic Service, supporting the titration and monitoring of medication and physical health observations for older adults.
Key Responsibilities- Manage a small caseload of patients as lead professional, overseeing medication titrations and monitoring.
- Liaise with General Practitioners regarding medication changes and ensure medications are supplied.
- Participate in the continuous improvement of the service and implementation of operational and clinical policies.
- Promote opportunities for local service development and demonstrate an understanding of our trust values.
- Travel across the Blackpool, Fylde, and Wyre areas to undertake home visits, utilizing a car with travel expenses provided.

Requirements- Registered Mental Health Nurse with degree-level qualifications or equivalent knowledge and experience.
- On-going registration with the NMC and compliance with regulatory bodies and codes of professional conduct.
- Experience in dementia and cognitive impairment, older adults, and medications used for dementia and cognitive problems.
- Knowledge of current mental health legislation and policy.
- Mentorship qualification and sign-off mentor qualification.
- Knowledge of current key NHS developments and policies and implications for service delivery.
- Experience in memory assessment services, medication titration, and facilitating groups.
