Job Summary
We are looking for a Practice Nurse who can work in an encouraging, supportive, and forward-thinking environment. The successful candidate will have experience in primary and community nursing, with a focus on long-term condition management. They will be able to work independently and as part of a team, and will have excellent communication and interpersonal skills.
Main Duties
- Assess, plan, develop, implement, and evaluate programmes to promote health and well-being.
- Implement and evaluate individual treatment plans for patients with a known long-term condition.
- Identify and manage as appropriate treatment plans for patients at risk of developing long-term conditions.
- Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Support patients to adopt health promotion strategies that encourage patients to live healthily.
- Deliver opportunistic health promotion using opportunities such as new patient health checks.
- Provide information and advice on prescribed or over-the-counter medication.
- Assess and care for patients presenting with uncomplicated wounds.
- Support and advise women requesting information relating to family planning needs and general health.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise, assess, and refer patients presenting with mental health needs.
- Implement and participate in vaccination and immunisation programmes.
- Advise, support, and administer vaccinations for patients travelling abroad.
- Promote and deliver evidence-based care for patients presenting with aural conditions.
- Assist senior partners in providing minor-surgery sessions.
Person Specification
- Relevant nursing/health degree.
- Diploma in an area of or in Long Term Chronic Disease Management.
- Membership of professional body.
- Minimum 3 years post-registration experience.
- At least 2 years recent primary and community nursing experience.
- Experience of nurse-led management of long-term conditions.
- Experience of implementing protocols and clinical guidelines.
- Experience of quality initiatives such as clinical benchmarking.
- Practice experience of working with others.
- Experience of using own initiative.
- Audit experience.
- IT skills.
- Working as part of a team.
