About the Role

Fleetwood Surgery is seeking a skilled and experienced Practice Nurse to join our dynamic team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our practice.

Key Responsibilities
  • Assess, plan, develop, implement, and evaluate programs to promote health and well-being.
  • Implement and evaluate individual treatment plans for patients with known long-term conditions.
  • Identify and manage as appropriate treatment plans for patients at risk of developing long-term conditions.
  • Prioritize health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
  • Deliver opportunistic health promotion using opportunities such as new patient health checks.
  • Provide information and advice on prescribed or over-the-counter medication on regimes, side effects, and interactions, as able to.
  • Assess and care for patients presenting with uncomplicated wounds.
  • Support and advise women requesting information relating to family planning needs and general health.
  • Support and manage health needs of women presenting for cervical cytology consultations.
  • Recognize, assess, and refer patients presenting with mental health needs in accordance with the National Framework for Mental Health.
  • Implement and participate in vaccination and immunization programs for both adults and children.
  • Advise, support, and, where appropriate, administer vaccinations for patients traveling abroad.
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Assist senior partners in providing minor-surgery sessions.
Person Specification
  • Relevant nursing/health degree.
  • Diploma in an area of or in Long Term Chronic Disease Management (Diabetes, COPD, Asthma, etc).
  • Membership of professional body.
  • Clinical supervision training and experience.
  • Independent/supplementary nurse prescribing qualification.
  • Minimum 3 years post-registration experience.
  • At least 2 years recent primary and community nursing experience.
  • Experience of nurse-led management of long-term conditions.
  • Experience of implementing protocols and clinical guidelines.
  • Experience of quality initiatives such as clinical benchmarking.
  • Practice experience of working with others.
  • Experience of using own initiative.
  • Audit experience.
  • IT skills.
  • Working as part of a team.
  • Knowledge of Primary Care Contract Requirements and application of service delivery to meet targets GMS Contract, QOF, Enhanced Services.
