Practice Nurse Position

Fleetwood Surgery is seeking a skilled Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our practice.
Key Responsibilities- Assess, plan, develop, implement, and evaluate programs to promote health and well-being.
- Implement and evaluate individual treatment plans for patients with known long-term conditions.
- Identify and manage treatment plans for patients at risk of developing long-term conditions.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Support patients in adopting health promotion strategies and applying principles of self-care.
- Deliver opportunistic health promotion using opportunities such as new patient health checks.
- Provide information and advice on prescribed or over-the-counter medication.
- Assess and care for patients presenting with uncomplicated wounds.
- Support and advise women requesting information relating to family planning needs and general health.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognize, assess, and refer patients presenting with mental health needs.
- Implement and participate in vaccination and immunization programs.
- Relevant nursing/health degree.
- Diploma in an area of or in Long Term Chronic Disease Management.
- Membership of a professional body.
- Minimum 3 years post-registration experience.
- At least 2 years recent primary and community nursing experience.
- Experience of nurse-led management of long-term conditions.
- Experience of implementing protocols and clinical guidelines.
- Experience of quality initiatives such as clinical benchmarking.
Fleetwood Surgery is an innovative and forward-thinking practice led by a partnership of 3 GPs. We serve a local population of approximately 6,500 patients and are committed to providing high-quality patient care.
We are a friendly, professional, and caring team who are passionate about delivering safe and effective care. Our excellent multi-skilled clinical team comprises of 4 GPs, Pharmacists, Pharmacy technicians, Practice Nurses, and HCAs.
We are an established Training Practice, and we pride ourselves in our commitment to teaching and supporting a variety of clinical educational placements.
