Job Summary

We are seeking an experienced Practice Nurse to join our team. The successful candidate will have a strong background in primary care and a passion for delivering high-quality patient care. The role will involve working closely with our clinical team to provide nursing assessments, develop treatment plans, and implement evidence-based care.

Key Responsibilities
  • Assess and manage patients with long-term conditions, including diabetes and respiratory management
  • Develop and implement individualized treatment plans to promote health and well-being
  • Provide education and support to patients on healthy lifestyle choices and disease management
  • Collaborate with the clinical team to ensure seamless care delivery and meet contractual obligations

Person Specification

We are looking for a Practice Nurse with:

Relevant nursing/health degree and diploma in long-term chronic disease managementMembership of a professional bodyMinimum 3 years post-registration experience and at least 2 years recent primary and community nursing experienceExperience of nurse-led management of long-term conditions and implementing protocols and clinical guidelinesStrong IT skills and ability to work as part of a team
