About the Role

We are seeking a skilled Clinical Practitioner to join our team as a Clinical Practitioner Specialist. This role will provide support to the ongoing development and mobilisation of integrated mental health services.

Main Responsibilities

This role will involve:

  • Providing follow-up care for service users who struggle to access care and support through traditional operating hours.
  • Working closely with colleagues in specialist community mental health services to ensure smooth transitions between teams and services.
  • Supporting service users through co-produced assessments and evidence-based interventions.
  • Supporting with processing referrals, triaging, and liaising with colleagues and carers in the provision of holistic support.

Requirements

To be successful in this role, you will need:

  • A recognised registered mental health qualification.
  • Substantial post-registration experience in the NHS.
