About The Role

Transforming Urgent Care in Devon

Practice Plus Group is seeking an ambitious and committed Advanced Clinical / Nurse Practitioner to join our multidisciplinary team in Devon. As part of our Out Of Hours (OOH) and Clinical Assessment Service (CAS), you will play a key role in delivering high-quality urgent care to patients.

Our Vision

We are committed to simplifying and speeding up the way patients receive care. By assessing patients, planning their care, administering treatment, and discharging or referring them to other healthcare professionals, you will be at the forefront of our Integrated Urgent Care model.

Your Key Responsibilities

  1. Work autonomously within your sphere of competency as part of a friendly and close-knit multi-skilled team.
  2. Use our electronic prescribing system to streamline patient care.
  3. Collaborate with GPs, registered nurses, UCPs, mental health and dental nurses to provide comprehensive care.
  4. Contribute to the development and implementation of our Integrated Urgent Care model.
