Nursing Associate/Assistant Practitioner

About the Role
This is an exciting opportunity to join our team as a Nursing Associate/Assistant Practitioner in our Community Nursing Team. As a key member of our team, you will play a vital role in delivering high-quality patient care and supporting our patients to achieve their goals.
Key Responsibilities
- Provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients.
- Work with other team members to embrace case management and the long-term conditions and Aging Well Agenda, preventing avoidable hospital admissions and facilitating appropriate earlier discharges into the community.
- Have excellent communication and observation skills, with the ability to work as part of a team and demonstrate flexibility to deal with the needs of unscheduled services.
- Manage own delegated workload and support others, with a willingness to undertake additional further qualifications and develop expertise in identified areas.

Requirements
- A full driving licence and use of a car for work purposes.
- Level 5 English and Maths.
- Registered Nursing Associate Qualification or Assistant Practitioner Qualification.
- Minimum 2 years Community Nursing experience.
