About the Role

This position is an exciting opportunity to join our team as a Nursing Associate/Assistant Practitioner in a community care setting. As a key member of our community nursing team, you will play a vital role in delivering high-quality patient care and supporting the success of our healthcare services.

Key Responsibilities
  • Provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients.
  • Work across healthcare teams to provide holistic assessments, wound care, and other essential services.
  • Collaborate with other team members to prevent avoidable hospital admissions and facilitate earlier discharges into the community.
  • Develop and maintain effective communication and observation skills to ensure patient-centered care.
  • Participate in case management and the long-term conditions and Aging Well Agenda.
Requirements
  • A full driving license and use of a car for work purposes.
  • Level 5 English and Maths.
  • Registered Nursing Associate Qualification or Assistant Practitioner Qualification.
  • Minimum 2 years community nursing experience.
