Nursing Associate/Assistant Practitioner

About the Role
We are seeking a highly skilled and dedicated Nursing Associate/Assistant Practitioner to join our Community Nursing Team at Berkshire Healthcare NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality patient care and contributing to the success of our community nursing services.
Key Responsibilities
- Provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients.
- Work across healthcare teams to provide holistic assessments, raise safeguarding concerns, and administer medications.
- Develop and maintain effective relationships with patients, carers, and healthcare professionals to ensure comprehensive care.
- Contribute to the development and implementation of care plans, ensuring patient-centered and person-centered approaches.
- Participate in multidisciplinary teams to enhance quality of service provision and promote patient safety.
Requirements
- Registered Nursing Associate Qualification or Assistant Practitioner Qualification.
- Minimum 2 years community nursing experience.
- Full driving license and use of a car for work purposes.
- Level 5 English and Maths.
