Community Mental Health Practitioner Role Overview

In the mid and south of Essex, we are currently seeking a highly skilled Mental Health Practitioner to join our innovative team in the Older Adult Admission Avoidance & Home Treatment Team. This is an exciting opportunity to make a meaningful difference in the lives of older adults with functional mental health issues.

Main Job Responsibilities

The successful candidate will be responsible for providing high-quality care and support to older adults with complex needs, focusing on admission avoidance and home treatment through a Home First approach. They will work closely with the community services and established Dementia Admission Avoidance services across Mid and South Essex.

Key Skills and Qualifications
  • A registered Mental Health Nurse or Occupational Therapist with a strong background in Older Adult Mental Health.
  • Demonstrable experience in using risk assessment tools and screening tools in dementia.
  • Ability to work independently and collaboratively as part of a dynamic team.
  • Familiarity with national policy and its implications for good mental health practice.
