Job Summary

We are seeking a highly motivated Care Coordinator to join our team in Mid and South Essex, working with older adults who have functional mental health issues. The successful candidate will play a vital role in supporting these individuals to meet their increased needs, while also helping to prevent admission avoidance and provide home treatment using a Home First approach.

About the Role

This is an exciting opportunity to join our new and growing team, working closely with community services and established dementia admission avoidance services across Mid and South Essex. As a Care Coordinator, you will be responsible for:

  • Supporting older adults with functional mental health issues to meet their needs and prevent hospital admissions
  • Working collaboratively with community teams, healthcare professionals, and other stakeholders to provide holistic care
  • Using your expertise to develop effective care plans, taking into account individual needs and preferences
  • Providing education and support to carers, family members, and other relevant individuals

Requirements

To be successful in this role, you will need:

  • A degree in a relevant field (e.g., nursing, occupational therapy)
  • Registration with the relevant professional body (e.g., NMC, HCPC)
  • Experience working with older adults and their families/carers
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
Salary and Benefits

The salary for this role is £35,392 - £42,618 per annum (pro rata for part-time hours), plus a range of benefits, including season ticket loans, NHS discounts, and excellent training facilities. We also offer a buying and selling annual leave scheme, allowing you to purchase or sell additional leave time.

Location

This role is based in Mid and South Essex, with easy access to major transport links. The Harland Centre is located in a convenient and accessible location, making it easy to commute to work.
