Community Health Specialist

At HCRG Care Group, we are committed to transforming health and care services. As a Band 5 Family Wellbeing Nurse, you will play a vital role in delivering community-based support to families across Essex. This part-time position is based in Braintree, where you will work alongside a multidisciplinary team to provide outcome-based support.
About the Role
You will be responsible for delivering the Healthy Child Programme, assessing health needs, and developing packages of care. Additionally, you will provide leadership and act as a positive role model, supporting colleagues and demonstrating excellent standards of care. You will also undertake mentorship responsibilities for pre-registration students and provide preceptorship to new staff members.
Key Responsibilities
- Deliver the Healthy Child Programme, including assessment of health needs and development of packages of care
- Provide leadership and support to colleagues, promoting excellent standards of care
- Mentor pre-registration students and provide preceptorship to new staff members
Requirements
- Nursing or Midwifery qualification
- Passion for helping children have the best possible start in life
- Confidence using IT systems, including Microsoft Office packages
- Valid UK driving license and access to a car for work use
