We're proud to be partnered with the Somerset NHS trust to support patients back into their own homes, offering fantastic opportunities to join our valued Home from Hospital care team as a Reablement Assistant.

As a Reablement Assistant, you'll work with customers to meet their individual needs, providing personal care duties, administering medication, and promoting health and well-being. You'll also be responsible for manual handling, using equipment where required, and encouraging customers to make connections with their local community.

We offer a competitive rate of £26,000 per year, paid mileage at 35p per mile, and ongoing training and support. You'll also have access to flexible working arrangements, including full-time and part-time options, and the opportunity to work in a collaborative and person-centred environment.

Your Responsibilities
  • Provide personal care duties, such as bathing and dressing, as required
  • Administer medication according to customer needs and preferences
  • Use manual handling techniques, with or without equipment, to assist customers
  • Promote customer health and well-being, encouraging them to engage in activities that promote independence and community links
