About the Role

This is a developmental role within our Mental Health Assessment team, where you will work closely with a multi-disciplinary team to provide specialist assessment and care planning for individuals experiencing mental health-related difficulties.

Key Responsibilities

  • Conduct comprehensive assessments of care needs and contribute to the formulation of individualized care plans.
  • Provide interventions to support individuals in meeting their agreed goals, working in partnership with service users, carers, and other healthcare professionals.
  • Coordinate and participate in the review of progress towards goals, utilizing recommended outcome measures.
  • Work autonomously in a community setting, making decisions and taking ownership of cases.

Person Specification

  • Registered Mental Health Nurse, Registered Social Worker, or Registered Occupational Therapist.
  • Experience of working with adults experiencing mental health-related difficulties.
  • Desirable: experience of working in a community setting and making decisions autonomously.
