About the Role

We are seeking a skilled Mental Health Practitioner to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with service users, carers, and other healthcare professionals to provide specialist assessment and care planning.

Key Responsibilities

  • Conduct comprehensive assessments of service users' mental health needs and develop personalized care plans.
  • Work collaboratively with the multidisciplinary team to provide interventions and support service users to achieve their goals.
  • Coordinate and participate in the review of progress towards goals, utilizing recommended outcome measures.
  • Provide specialist advice and guidance to colleagues and other healthcare professionals.

Person Specification

  • Registered Mental Health Nurse, Registered Social Worker, or Registered Occupational Therapist.
  • Experience of working with adults experiencing mental health-related difficulties.
  • Desirable: experience of working in a community setting and making decisions autonomously.
