About the Role
As a Discharge Planning Specialist, you will be responsible for managing a caseload of patients and working closely with the multidisciplinary team to develop and implement discharge plans. You will ensure that all necessary steps are taken to prepare patients for discharge, including arranging assessments, coordinating care packages, and communicating with patients, carers, and healthcare professionals.
Key Responsibilities
The key responsibilities of this role include:
- Managing a caseload of patients and developing discharge plans in collaboration with the MDT
- Coordinating care packages and arranging assessments with external agencies
- Communicating with patients, carers, and healthcare professionals to ensure a smooth transition
- Escalating any issues or concerns to relevant team leaders or clinical leaders
Requirements
To be successful in this role, you will need to have:
- A NVQ 3 or Diploma Advanced Apprenticeship Level 3 Health & Social Care or equivalent
- Practical experience in a healthcare setting
- The ability to work independently and prioritize a varied workload
Estimated Salary Range
The estimated salary range for this role is £30,000 - £40,000 per annum, depending on experience.
