Role Overview

We are seeking a highly skilled Clinical Pharmacist to support GP practices with medicines-related tasks and contribute to improving patient outcomes.

  • Utilize clinical systems (Emis and S1) to manage patient requests and conduct medication reviews.
  • Collaborate with colleagues to resolve medication-related issues and provide expert clinical advice.
Key Responsibilities
  • Support the delivery of pharmacist services across the PCN, ensuring effective use of skill mix and resources.
  • Develop and implement quality improvement initiatives to optimize medicines use and patient care.
Requirements
  • Completed CPPE Pathway or currently enrolled in the program.
  • Experience working in primary care, preferably in a GP practice setting.
