Job Title: Support Worker - Housing & Independence Service

We are seeking a dedicated and compassionate Support Worker to join our Housing & Independence Service team. As a Support Worker, you will play a vital role in supporting care experienced young people in their transition to independence.

Key Responsibilities:

  • Provide emotional support and guidance to young people in their transition to independence
  • Develop and implement individualized support plans to meet the unique needs of each young person
  • Work collaboratively with the Housing & Independence Service team to ensure a seamless transition for young people
  • Provide information and support to young people on key life skills, such as budgeting, cooking, and cleaning
  • Assist young people in accessing community resources and services

Requirements:

  • Experience working with care experienced young people
  • Ability to develop and implement individualized support plans
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Knowledge of key life skills and community resources
