Job Summary

We are seeking a highly motivated and enthusiastic Band 5 nurse to join our Community Nursing Team as a Staff Nurse within the South Tees Hospitals NHS Foundation Trust. This role offers the opportunity to provide high-quality, patient-centered care to individuals in their own homes and residential settings.

Main Responsibilities

The successful candidate will be responsible for:

  • Providing clinical care and support to patients with complex and high-acuity needs
  • Working closely with GP practices and other partners to prevent unnecessary hospital admissions
  • Delivering effective evidence-based care to a defined demographic population with diverse healthcare and social needs
  • Collaborating with the District Nurse Sister and Clinical Lead to develop and implement innovative practice
  • Participating in leadership and improvement training programs to enhance leadership effectiveness and quality improvement skills
Requirements

To be successful in this role, the candidate must:

  • Be registered as fit to practice by the Nursing and Midwifery Council (NMC)
  • Hold a Diploma or Degree in Adult Nursing
  • Have experience of multi-disciplinary working and a commitment to team objectives
  • Be able to use initiative to make decisions within their sphere of work and recognize when assistance is required
  • Have good information technology skills
