About the RoleThe successful candidate will be responsible for delivering clinical care, innovative practice, and support to the District Nurse Sister and Clinical Lead. This role requires an experienced clinician with team working and communication skills, delivering effective evidence-based care to a defined demographic population with diverse healthcare and social needs.
Key Responsibilities- Provide clinical care and support to patients in their own homes and residential settings.
- Work closely with GP practices and other partners to support patients with complex and high-acuity needs.
- Develop and implement care plans that meet the individual needs of patients.
- Collaborate with other healthcare professionals to ensure seamless care delivery.
- Participate in quality improvement initiatives to enhance patient outcomes and experience.
- Registered as fit to practice by the Nursing and Midwifery Council (NMC).
- Diploma or Degree in Adult Nursing.
- Experience of multi-disciplinary working.
- Good information technology skills.
