Job Summary
We are seeking an experienced and motivated nurse to join our Community Nursing Team as a Staff Nurse within the South Tees Hospitals NHS Foundation Trust. This role will provide high-quality, patient-centered care to individuals in their own homes and residential settings.
Main Responsibilities
The successful candidate will be responsible for providing clinical care, innovative practice, and support to the District Nurse Sister and Clinical Lead. This will involve working closely with GP practices and other partners to support patients with complex and high-acuity needs, preventing unnecessary hospital admissions.
Requirements
- Registered as fit to practice by the Nursing and Midwifery Council (NMC)
- Diploma or Degree in Adult Nursing
- Experience of multi-disciplinary working
- Commitment to the needs of the service, including a flexible and adaptable approach
