Community Staff Nurse

We are seeking a highly motivated and experienced Staff Nurse to join our Community Nursing Team as a Community Staff Nurse within the South Tees Hospitals NHS Foundation Trust. This role offers an exciting opportunity to provide high-quality, patient-centered care to individuals in their own homes and residential settings.
Main Responsibilities- Provide clinical care and support to patients with complex and high-acuity needs, working closely with GP practices and other partners to prevent unnecessary hospital admissions.
- Develop and implement innovative practice and care plans, utilizing evidence-based approaches to improve patient outcomes.
- Collaborate with the District Nurse Sister and Clinical Lead to develop and implement team goals and objectives, promoting a culture of excellence and continuous improvement.
- Participate in quality improvement initiatives, contributing to the development and implementation of strategies to enhance patient care and service delivery.
- Develop and maintain strong relationships with patients, families, and healthcare professionals, promoting effective communication and collaboration.
- Registered as fit to practice by the Nursing and Midwifery Council (NMC).
- Diploma or Degree in Adult Nursing.
- Experience of multi-disciplinary working and a commitment to team objectives.
- Ability to use initiative to make decisions within the scope of work and recognize when assistance is required.
- Good information technology skills.
