Clinical Lead Band 6 Position

We are seeking a highly skilled and experienced Clinical Lead to join our team at Midlands Partnership NHS Foundation Trust. As a Clinical Lead, you will be responsible for managing a team and supporting the delivery and oversight of the 8 care process checks for people with diabetes.
About the RoleThis is a dynamic position that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in providing health care screening, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.
Key Responsibilities- Assess new service user referrals and care & support needs, having oversight of and undertaking delivery in relation to the 8 care process checks.
- Use expert knowledge and judgement to take into account the range of care/support packages and treatment options available, considering each service user's personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
- Lead in organising the induction of clinical supervision of students and new team members and act as a role model to the team.
- Must be capable of cost and time efficient travel across a dispersed service area, this post will involve working across MPFT Staffordshire. A car user with a full driving licence would be required due to areas covered in this role.
Qualifications:
- Degree in nursing or equivalent experience.
Desirable:
- Evidence of further clinical study.
Experience:
- Relevant experience to the role including significant experience working in a community setting.
- Experience and knowledge of working with patients with Diabetes.
