Key Responsibilities- Develop and implement personalized support plans in consultation with residents, ensuring their needs and goals are met.
- Promote person-centered planning and involvement, empowering residents to make informed decisions about their lives.
- Maintain accurate and detailed records of resident interactions, progress, and support plans.
- Liaise with other services and agencies to ensure residents receive comprehensive support and advocacy.
- Support residents in daily tasks, such as shopping, appointments, and meal preparation.
- Contribute to a safe and supportive living environment, promoting resident well-being and independence.
- Collaborate with colleagues to ensure seamless support and care for residents.
We are looking for a passionate and dedicated individual who shares our commitment to supporting young people. You will have:
- Experience in a support worker setting, preferably in residential childcare.
- A strong passion for improving young people's lives and a desire to develop a vocation in support work.
