We are looking for dedicated and compassionate people. As Care Assistant / Support Workers you will be passionate about your job, enjoy delivering one to one support and can build positive relationships with one of our service users.
As a Care Assistant and Support Worker you will play a vital role in providing compassionate care and assistance to an individual, ensuring their physical, emotional, and social well-being.
**Responsibilities**:
1. **Personal Care**: Assist client with activities of daily living, including prompting with bathing, dressing, grooming, and mobility support.
2. **Medication Management**: Administer prescribed medications according to the care plan and ensure clients take medication as prescribed.
3. **Meal Preparation**: Plan and prepare nutritious meals based on dietary restrictions and preferences, ensuring proper hydration and nutrition.
4. **Housekeeping**: Perform light housekeeping tasks, such as cleaning, laundry, and maintaining a safe and tidy living environment.
5. **Companionship**: Provide emotional support, engage in conversation, and participate in recreational activities to promote social interaction and mental well-being.
6. **Transportation**: Accompany clients to medical appointments, grocery shopping, and other necessary outings as required.
7. **Documentation**: Maintain accurate records of care provided, changes in condition, and any incidents or observations as per Phoenix Care protocols.
8. **Safety**: Ensure the safety and security of clients by identifying and addressing potential hazards and emergencies promptly.
9. **Communication**: Collaborate with healthcare professionals and other team members to coordinate and deliver comprehensive care.
10. **Respect for Dignity**: Treat clients with respect, dignity, and empathy, recognizing their individuality.
**Experience**:
- Previous experience in a similar role in healthcare, social services, or caregiving is highly desirable.
- Familiarity with working in community-based setting is a plus.

- Excellent communication and active listening skills.
- Empathy and compassion when working with clients and their families.
- Ability to build rapport and establish trust with clients.
**Physical and Emotional Resilience**:
- Physically capable of assisting with lifting and maneuvering clients when necessary.
- Ability to remain calm and composed in stressful or emotional situations.

- Proficient in providing personal care, including bathing, grooming, and dressing.
- Ability to assist with medication management as per organizational policies.
**Teamwork**:
- Collaborative and able to work effectively within a team of healthcare professionals.
- Willingness to support colleagues and share responsibilities.
**Problem Solving**:
- Capacity to identify and address problems or concerns related to client care.
- Ability to adapt to changing circumstances and make quick, informed decisions.
**Organization and Time Management**:
- Effective at prioritizing tasks and managing time to ensure clients' needs are met.
- Attention to detail in maintaining accurate records and reports.
**Other Requirements**:
**Criminal Background Check**: Willing to undergo and pass a criminal background check as required by law.
**Valid Driver's License**: A valid driver's license is desirable.
**References**: Able to provide references from previous employers or relevant individuals.
