About the Role

KINGSTON HEALTH is seeking an experienced Care Navigator to join their team. As a Care Navigator, you will play a vital role in supporting patients in accessing the appropriate healthcare services.

Key Responsibilities

  • Provide excellent patient care and support, ensuring that patients receive the best possible service.
  • Use initiative and discretion to resolve patient queries and concerns.
  • Work collaboratively with the practice team to ensure seamless delivery of services.
  • Maintain accurate and up-to-date records, adhering to confidentiality and data protection policies.
  • Participate in training and development programs to improve skills and knowledge.

Requirements

  • GCSE Grade A to C or equivalent.
  • NVQ level 2 or above in Customer Service.
  • Previous experience in a healthcare setting, preferably in primary care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
