About the Opportunity

We are seeking an experienced Band 6 Community Practitioner to join our team in North Wales, providing community rehabilitation services for the Mental Health and Learning Disability Division.

The successful candidate will have a professional qualification in adult mental health and experience in working with service users who require long-term management in the community.

A key aspect of this role is working as a care coordinator under the Mental Health (Wales) Measures Framework, managing a caseload and facilitating the repatriation of services users back to their home area.

This is an excellent opportunity for a registered practitioner such as a mental health nurse or social worker to make a meaningful contribution to our community.

Key Responsibilities:

  • Work collaboratively with multidisciplinary teams to provide intensive community rehabilitation
  • Manage a caseload and facilitate the repatriation of services users back to their home area
  • Assess and develop individualised care plans to meet the unique needs of service users
  • Promote equality and diversity in all aspects of work
  • Contribute to the development of policies and procedures that reflect best practice in community rehabilitation
