Community Care Coordinator

About the Role:
The Community Mental Health Team is seeking a skilled Mental Health Nurse to work within an established integrated, multi-disciplinary team. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.
Main Duties:
- To provide high-quality care and support to individuals with severe and enduring mental health problems.
- To work collaboratively with service users, carers and partner agencies to deliver effective care and interventions.
- To assess and manage clinical risk, and develop strategies to mitigate potential risks.
- To maintain accurate records and reports, and contribute to quality improvement initiatives.
Requirements:
We are looking for a highly motivated and organised individual with excellent communication and interpersonal skills. You should have experience working in a mental health setting and possess a strong understanding of the Mental Health Measure (Wales). Estimated salary: £40,000 - £50,000 per annum.
