We are seeking a skilled and compassionate Care Leavers Team Coordinator to join our 18+ Care Leavers Service Team in Birmingham.

Job Summary: This is a full-time position with a salary up to £35,336 per annum. As a Care Leavers Team Coordinator, you will work collaboratively with the team to support care leavers in their transition to adulthood.

Key Responsibilities:

  • Team Collaboration: Act as a valued team member, contributing to the Service's development and growth.
  • Caseload Management: Effectively manage your caseload, providing individualised support to young people and groups.
  • Youth Support: Deliver programmes to assist young people in transitioning to independent living, focusing on life skills, education, and employment.
  • Procedure Implementation: Implement and adhere to Service and Directorate procedures, ensuring best practice is followed at all times.
  • Holistic Approach: Adopt a holistic approach to meeting individual needs, promoting self-esteem through positive achievement and empowerment.
  • Staff Support: Advise staff on preparation and support needs of care leavers, promoting services for these groups and maximising resources.
  • Collaboration: Work closely with other agencies to ensure effective support for care leavers, promoting interagency working and partnership building.
  • Evaluation and Reporting: Monitor and evaluate Service work, providing regular reports and recommendations for improvement.
