Care and Wellbeing Coordinator Role

We are seeking an experienced Care and Wellbeing Coordinator to join our team at Gold Care Homes. As a key member of our multidisciplinary team, you will play a vital role in ensuring residents receive comprehensive care and support.

Duties and Responsibilities:

  • Establish and maintain effective relationships with residents, families, and healthcare professionals.
  • Conduct thorough assessments and develop tailored care plans.
  • Coordinate care services, including medication management and medical procedures.
  • Supervise junior staff members and provide guidance and support.
  • Collaborate with colleagues to ensure seamless care transitions and continuity.
