Care Coordinator Specialist

About This Role:
We are seeking an experienced Care Coordinator to join our team at House of Care Services LTD. The ideal candidate will have a strong background in care coordination, excellent communication skills, and a commitment to delivering exceptional care.
Key Responsibilities:
- Staff Supervision: Conduct regular staff supervisions and lead by example to ensure high standards of care are maintained.
- Care Planning: Develop and implement effective care plans that meet the needs of our clients.
- Communication: Build strong relationships with clients, families, and other healthcare professionals to ensure seamless care delivery.
Salary and Benefits:
We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, plus a range of benefits including comprehensive training, career development opportunities, and a supportive work environment.

- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Working Hours:
This is a full-time role, working 37.5 hours per week from one of our locations.
