About the Role

We are seeking a skilled and compassionate Mental Health Practitioner to join our team at Midlands Partnership NHS Foundation Trust. As a Band 6 Mental Health Practitioner, you will play a vital role in providing specialist assessment and care planning to individuals experiencing mental health crises or requiring extra support.

Working in a multi-disciplinary team, you will collaborate with internal and external agencies to deliver high-quality clinical services to patients and their families. Your expertise will be invaluable in developing and implementing therapeutic interventions, in accordance with agreed care plans, to support individuals experiencing mental health-related difficulties.

Key Responsibilities
  • Provide specialist assessment and care planning in conjunction with service users and carers.
  • Deliver therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health-related difficulties.
Person Specification
  • Registered Mental Health Nurse/ Registered Social Worker/ Registered Occupational Therapist.
  • Experience of working in a community setting and making decisions autonomously.
