Job Title: Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care in the community, working closely with patients, families, and healthcare professionals to promote health, prevent illness, and manage long-term conditions.

About the Role

Key Responsibilities
  • Assess patients' needs and develop individualized care plans
  • Provide high-quality patient care, including wound management, medication administration, and health education
  • Work collaboratively with patients, families, and healthcare professionals to promote health, prevent illness, and manage long-term conditions
  • Contribute to the development and implementation of community-based healthcare services
  • Participate in quality improvement initiatives and contribute to the continuous improvement of patient care
Requirements

To be successful in this role, you will need to have:

Registered Nurse qualification and current registration with the NMCExperience of working in a community setting, preferably in a similar roleExcellent communication and interpersonal skillsAbility to work independently and as part of a teamFlexibility and adaptability to meet the changing needs of patients and the serviceWhat We Offer
