We are seeking a skilled and compassionate Community Nurse to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care in the community, working closely with patients, families, and healthcare professionals to promote health, prevent illness, and manage long-term conditions.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and expertise as a Community Nurse. You will be responsible for assessing, planning, implementing, and evaluating patient care, working in partnership with patients, families, and healthcare professionals to achieve the best possible outcomes.
Key Responsibilities- Assess patients' needs and develop individualized care plans
- Provide high-quality patient care, including wound management, medication administration, and health education
- Work collaboratively with patients, families, and healthcare professionals to promote health, prevent illness, and manage long-term conditions
- Contribute to the development and implementation of community-based healthcare services
- Participate in quality improvement initiatives and contribute to the continuous improvement of patient care
To be successful in this role, you will need to have:
