Role Specifics

We are looking for a Community Nursing Coordinator Lead to manage the newly unified Community Nursing team, which has recently merged from separate entities. The role involves direct supervision of healthcare professionals, including Community Nurses, Occupational Therapists, and Physiotherapists. You will be responsible for monitoring team performance and ensuring optimal resource utilization to achieve significant outcomes.

Your focus will be on maintaining service quality, fostering innovation in educational delivery, and supporting safeguarding processes within care settings to ensure excellence in service provision.

The team operates across a broad geographical area, and the successful candidate's base location will be determined based on their preferences and qualifications. Travel across the service area will be necessary to meet the demands of the role, which includes both remote and in-person engagements with care providers.

Opportunities for conducting audits and evidence-based initiatives will be available to enhance the skills and knowledge of community colleagues, aligning with our goal of delivering timely and appropriate care to our aging population.

You will collaborate with subject matter experts to align resources and work across various healthcare groups to adhere to strategic objectives.
