We are seeking a skilled and compassionate Clinical Care Specialist to join our team at Central London Community Health Trust. This exciting opportunity allows you to make a positive impact on the lives of patients in need of community nursing care.

Key Responsibilities- Deliver high-quality clinical care to patients in their homes, working closely with our community nursing team to provide safe, effective, and holistic care.
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with multidisciplinary teams, including GPs, hospitals, and social services, to ensure seamless care coordination.
- Maintain accurate and comprehensive records of patient interactions, treatments, and outcomes.
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or equivalent qualification.
- Experience working in a healthcare setting, preferably in community nursing.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
- Able to work autonomously and as part of a team, with a flexible approach to meeting changing priorities and deadlines.
- A competitive salary range of £25,000 - £30,000 per annum, depending on experience.
