Healthcare Liaison Officer

We are looking for a dedicated Healthcare Liaison Officer to join our team in South Birmingham Alliance PCN. This is a challenging yet rewarding role that requires strong communication and organizational skills.
As a key member of our MDT, you will be responsible for coordinating the work of healthcare professionals and non-clinical staff, ensuring effective collaboration and communication. You will manage reporting requirements, act as administrative support to the practice safeguarding lead, and maintain IT-based information systems.
You will also be involved in patient identification, triaging requests for appointments, and supporting the completion of new referrals. You will work closely with service providers and clinicians to identify frequent flyers and new service users, utilizing risk stratification tools.

Salary: £25,593 - £27,649 per annum (WTE and pro rata if PT)
